Service Overview
Garth is a small, characterful residential home located in the heart of Denbigh, North Wales. Garth provides specialist supported living for adults aged 18 and over with learning disabilities and mental health conditions. With accommodation for just two residents, the service offers a highly personalised and intimate environment where individuals receive bespoke care tailored to their unique needs, preferences, and aspirations.
The home is designed to promote independence, dignity, and community inclusion, with a strong emphasis on person-centred planning, active support, and positive risk-taking. Residents are encouraged to take control of their lives, develop life skills, and engage meaningfully with their local community. Admission Criteria
Garth welcomes individuals who meet the following criteria:
- Age: 18 years and over
- Needs: Individuals with a diagnosis of learning disability, mental health condition, or complex needs
- Referral Process: All referrals are made through MHC’s Placement Team
- Assessment: A comprehensive pre-admission assessment is carried out, covering physical, emotional, cultural, and communication needs, as well as aspirations and risks
- Involvement: The person, their family, advocates, and professionals are actively involved in the decision-making process
- Compatibility: Staff matching tools are used to ensure shared interests and emotional compatibility
- Funding: Admission is subject to funding approval from the relevant authority
- Trial Period: A 12-week placement is offered to assess suitability, followed by a full review with all stakeholders

- Personal care: Assistance with hygiene, dressing, and health routines (as needed)
- Education and employment: Support to access college courses, training, volunteering, and work placements
- Social and leisure activities: Help planning and participating in hobbies, outings, and community events
- Health and well-being: Access to GP services, medication reviews, and referrals to specialists
- Communication support: Use of easy-read materials, communication passports, and advocacy services
- Person-Centred Planning (PCP): A detailed care plan developed with the resident, reviewed regularly to reflect changing needs
- Keyworker meetings: Monthly discussions to review progress, raise concerns, and update plans
- Emotional support: Staff are trained to provide compassionate, understanding care and build trusting relationships

Garth is a two-storey, mid-terrace house with a warm and homely feel. It offers:
- Two private bedrooms for residents
- Shared kitchen/Diner, and bathroom
- Two private lounge areas for residents
- Rear patio with seating and a designated covered and uncovered seated area
- Small front garden
- Telephone and Wi-Fi access throughout the home
- Regular maintenance by certified professionals (Gas Safe engineers, electricians, fire safety specialists)

Registered Manager
William Holmes is the Registered Manager at Glasfryn & Garth.
He has over 11 years of experience in the health and social care sector, all of which have been with MHC. Throughout his career, William has dedicated himself to working with individuals who have Mental Health Conditions and Learning Disabilities.
He began his journey with MHC as a Support Worker and has progressed within the organisation, accumulating extensive experience in assisting individuals with complex needs. In 2024, he was awarded a Bronze Medal at the Wales Care Awards and was recognised as the North Wales Higher Apprentice of the Year by Coleg Llandrillo. His objective is to ensure that residents enjoy the ‘best possible life’ by providing them with optimal opportunities in a supportive environment.
