Service overview
New Hall Care Home with Nursing can support up to 12 men who may require nursing care and support to enable them to achieve their potential by enhancing wellbeing, independence and quality of life.
The staff team support people to reach their goals by making the person central to their journey, in partnership with families and other stakeholders.
The care home is a detached property set within extensive grounds including woodland and an ornamental lake. This provides plenty of space, peace, and tranquillity, combined with good access to community facilities.
Admission Criteria
- Men only aged 18 and over
- Diagnosis of mental health condition and/or learning disability or other associated conditions or complex needs
What we do
We provide a step-down service for individuals moving from acute services or a step-up service from community-based services where it is felt robust support is required.
New Hall employs both Registered Mental Health Nurses (RNMH/RMH) and Registered Learning Disability Nurses (RNLD) who offer clinical care and expertise to support residents with mental health problems which include:
- Schizophrenia
- Mood disorders
- Personality Disorders (PD)
- Complex behaviours from trauma
- Acquired Brain Injury (ABI)
- Attention Deficit Hyperactivity Disorder (ADHD)
Registered General Nurses (RGN) employed by the care home have experience of mental health and complex needs and specialist training is provided. The nursing team support residents in the areas of weight management, diabetes, skin conditions, epilepsy and other physical health needs.
The service has psychiatry, psychology, and social work available on request, subject to commissioning and admission arrangements.
All staff at New Hall are trained in supporting people who have a forensic history and those with behaviours seen as challenging.
How we do it
The care and support we provide is tailored to meet individual needs. A comprehensive pre-admission assessment is always completed to ensure the team and the environment at New Hall can meet those needs. If we can’t meet someone’s needs, we will provide clear feedback about the reasons for this.
Active Support is used at New Hall, this model of support changes ‘caring for’ to ‘working with’ and promotes independence and supports people to take an active part in their own lives. The support given to the person is also active. Active Support enables people with complex needs to live ordinary lives.
Individual person-centred plans are formulated, prior to admission and then reviewed and developed regularly. The plan includes personal preferences, choices, goals, lifestyle, specific support needs, along with risk assessments.
The Multidisciplinary Team (MDT) is made up of the person being supported, their family carers and the staff team.
All residents are offered an annual health check with a Health Action Plan (HAP) to identify health concerns at an early stage and promote actions to address identified health needs.
Facilities
The environment at New Hall lends itself to giving people the choice to take part in activities or have time alone. There is an activities room where a personalised activity schedule accommodates each person’s preferences. All residents are supported to create their own bespoke activity plan through the communication method most appropriate to them.
Together, residents and staff review the activity schedule regularly.
Residents can access community facing activities and amenities such as education at local colleges, work-based placements, leisure activities and visits to places of interest.
New Hall provides opportunities for residents to work in a volunteering capacity on projects such as maintenance and wildlife conservation in the locality. The aim is to encourage and empower residents to explore a range of vocational, educational, and recreational activities in order that they can move on to a residential placement and eventually a community-based home.
The Surrounding Area
New Hall is a large community facing service on the outskirts of the village Ruabon in Wrexham, North Wales.
New Hall is within walking distance of the local village of Ruabon with local amenities which include a small supermarket, fish & chip shop, café, bakery, library, post office and newsagents.
There are vehicles available to support people to visit nearby Wrexham, which provides a wide range of shopping, leisure and social amenities including colleges, gyms, and numerous sports facilities.
There are several places of interest near New Hall such as the famous Frontcysyllte Aqueduct, located on a world heritage site near Ruabon, Garden Wood, Chester, Llangollen and Tŷ Mawr Country Park.
Transport connections are available via bus routes and the local train station to Wrexham, Chester, Llangollen and Holyhead, and wider afield.
Registered Manager
Registered Manager, Jessica Edwards
Jessica is the Registered Manager of New Hall Care Home with Nursing. She is registered with Social Care Wales and is a qualified Mental Health Nurse with several years of experience working in the healthcare sector, both within the NHS and private settings.
Throughout her career, Jessica has contributed to the development of new services. She has completed extensive training in Dialectical Behaviour Therapy (DBT) therapy and Positive Behaviour Support (PBS). Jessica is an accredited Foundations for Safer Care trainer through the British Institute of Learning Disabilities (BILD), delivering training to MHC staff in order that they provide the highest quality of care.
She supports the service and its residents by role-modelling open and transparent practices. She fosters a positive, lessons-learned culture and creates an environment that promotes learning and development for both residents and staff.
