Service Overview
Glasfryn Terrace is a warm and welcoming residential care home in Denbigh, North Wales, offering personalised support for adults aged 18 and over with mental health conditions and learning disabilities.
With space for just three residents, we provide a calm, homely environment where individuals receive tailored care that promotes independence, dignity, and well-being
Admission Criteria
- Adults aged 18 and over are eligible for admission.
- We support individuals with mental health conditions or learning disabilities, including those with complex needs.
- All referrals must go through MHC’s dedicated Placement Team.
- A detailed pre-admission assessment is carried out to understand each person’s care needs, goals, preferences, and any risks.
- We work closely with the individual, their family, advocates, and professionals to ensure the best possible support plan.
- We use a staff matching tool to ensure shared interests and the right support team for each person.
- Admission is subject to funding approval from the relevant local authority or commissioning body.
- A 12-week trial placement is offered to ensure Glasfryn Terrace is the right fit before confirming a long-term stay.

The Support Delivered
At Glasfryn Terrace, every resident receives personalised, compassionate support designed to help them live as independently and confidently as possible. Our team works closely with each person to understand their needs, preferences, and goals, creating a tailored care plan that promotes well-being, independence, and a sense of belonging. Whether you’re looking for supported living for adults with mental health needs or learning disabilities in North Wales, we’re here to help.
Here’s what support includes:
- A core package of 63 hours of support per week, with additional hours available based on individual needs.
- Help with daily living skills like cooking, cleaning, laundry, shopping, budgeting, and managing medication.
- Personal care support, including assistance with washing, dressing, and personal hygiene if needed.
- Encouragement to take part in education, training, volunteering, or work placements to build confidence and skills.
- Support to enjoy social activities, hobbies, and community events that match personal interests.
- A Person-Centred Plan (PCP) developed with each resident to reflect their goals, preferences, and support needs.
- Regular keyworker meetings and reviews to ensure care remains responsive and up to date.
- Help accessing healthcare services, including GP appointments, medication reviews, and specialist referrals.
- Access to advocacy services and communication support, including easy-read materials and communication passports.
- A focus on independence, choice, and inclusion, helping residents feel empowered and part of their community.
Facilities
- Two adjoining two-storey terraced houses (No. 2 and No. 3 Glasfryn Terrace)
- Three bedrooms for residents (1 in No. 2, 2 in No. 3)
- Staff sleep-in room (No. 3) and office space (No. 2)
- Fully equipped kitchens and comfortable lounges in both houses
- Bathrooms:
– No. 2: Bathroom with both a walk-in shower and a bath
– No. 3: Shared bathroom - Outdoor space:
– Covered and uncovered seating areas
– Split-level lawn with mature plants
– Large decked area for relaxing or socialising - Telephone and internet access available throughout the home
- Regular maintenance by certified professionals (e.g. Gas Safe engineers, electricians)

Registered Manager
William Holmes is the Registered Manager at Glasfryn & Garth.
He has over 11 years of experience in the health and social care sector, all of which have been with MHC. Throughout his career, William has dedicated himself to working with individuals who have Mental Health Conditions and Learning Disabilities.
He began his journey with MHC as a Support Worker and has progressed within the organisation, accumulating extensive experience in assisting individuals with complex needs. In 2024, he was awarded a Bronze Medal at the Wales Care Awards and was recognised as the North Wales Higher Apprentice of the Year by Coleg Llandrillo. His objective is to ensure that residents enjoy the ‘best possible life’ by providing them with optimal opportunities in a supportive environment.
